Welcome to Layton Elementary!

Please complete the following forms, print and bring to the office.  If you need paper copies please contact the school at 801-402-3500. 

Student Demographic Form- Complete, Print and Sign
Guardianship Status Form- Complete, Print and Sign
Immunization Form - Print and Complete
Proof of Residency - Students must enroll in the school within the boundary where they reside. Please refer to the Proof of Residency form for accepted documentation.
Affirmation of Residence- if you are living with another family, relative, etc. the affirmation of residence must be filled out as part of the registration process

Homeless Assistance Act Questionnaire - (McKinney-Vento Eligibility) Answer the two questions on the form.

Volunteering at the school?  Complete a District Background check, call 801-402-5722 to set up an appointment

 

To complete the enrollment process we will also need:
Original Birth Certificate (office will make a copy)
Original Immunization Records

(Immunization Requirements)
Guardian photo ID
Two proofs of residency

(Example: ID, utility bill, medical bill, bank statement)
